What It Covers
Understanding Medicare Part D
Medicare Part D offers prescription drug coverage to help cover the costs of medication. This coverage is optional, but it is highly regulated by the federal government and provided through private insurance companies approved by Medicare.
You can obtain Part D coverage in two ways: as a standalone Prescription Drug Plan (PDP) that pairs alongside Original Medicare, or bundled directly within a Medicare Advantage plan. Each plan utilizes a formulary, which is a structured tier list of medications they agree to cover. Because these formularies can vary between plans, it is important to review how specific necessary medications align with a plan’s structure during designated enrollment periods.
The penalty
Understanding the Medicare Part D Late Enrollment Penalty
This is the single most expensive misunderstanding in Medicare, and it’s entirely avoidable.
If you do not enroll in Medicare Part D when you are first eligible and go without creditable prescription drug coverage for 63 days or more, you may have to pay a late enrollment penalty if you enroll later. This penalty may be added to your monthly Part D premium and can continue as long as you have Medicare prescription drug coverage. Reviewing your options when you are first eligible can help you avoid potential additional costs.
Already have drug coverage somewhere else?
Coverage through an employer, a union, the VA or TRICARE often counts as creditable, which means no penalty for delaying. Your plan has to tell you in writing each year whether it counts. That notice is worth keeping. Call me, and we’ll check what you’ve got.

Questions
Questions about Part D.
What is the late-enrollment penalty?
If you go without creditable prescription coverage for long enough after you’re first eligible for Part D, and later decide to enroll, an amount is added to your monthly premium. You keep paying it for as long as you have Part D. It grows with the length of the gap, so the longer the delay the larger it is.
What counts as creditable coverage?
Drug coverage that Medicare considers at least as good as a standard Part D plan, often coverage from an employer, a union, the VA, or TRICARE. If you have one of those, you can usually delay Part D with no penalty. Your plan is required to tell you each year whether it’s creditable; keep that notice.
I don’t take any medications. Do I still need it?
That’s the situation the penalty exists for. Signing up when you’re first eligible, even with nothing to fill, keeps the door open at no penalty later, and health changes without warning. It’s worth a conversation rather than a default assumption either way.
Can I have Part D with a Medicare Advantage plan?
Usually the Advantage plan already includes it, and if it does you generally can’t add a separate Part D plan. If your Advantage plan doesn’t include drug coverage, that’s a specific situation worth checking with me before you add anything.
Can I change plans if my prescriptions change?
Plans can change their drug lists each year, and so can your prescriptions, which is why it’s worth re-checking during the Annual Enrollment Period each autumn rather than letting a plan roll over unexamined. That review is part of what I do for clients, at no cost.
How Will Prescription Coverage Work With My Medicare Plan?
Whether you have Original Medicare with a Medicare Supplement, are considering Medicare Advantage, or are exploring your options, it’s important to understand how prescription drug coverage fits into your plan.
